Hospital Discharge

Being discharged from hospital after an acute illness, injury or surgery can be overwhelming.  OT4Life can make this process easier and safer by:

• liaising with the discharge planning team at the hospital to ensure a safe discharge plan
• conducting a pre-discharge home safety assessment 
• facilitating equipment provision including delivery and installation
• recommending adjustments to the equipment
• facilitating home modifications
• collaborating with renovators/contractors
